What Is Robotic Adenomyosis Surgery? In Which Cases Is It Performed?

Operations are performed at Acıbadem Altunizade Hospital with the da Vinci robotic surgical system.
Request an AppointmentAdenomyosis is the condition in which the tissue called the ‘endometrium’, which lines the inside of the uterus, is found within the muscle tissue (myometrium), another layer of the uterus. Adenomyosis can generally cause painful periods, pain outside the menstrual period, infertility and recurrent miscarriages. It may occur throughout the whole uterus or be seen in only a single area.
Although adenomyosis is regarded as a condition that is not life-threatening, it affects women’s quality of life adversely because it frequently causes pain and leads to heavy periods. While it is not known exactly why this condition occurs, according to the research carried out oestrogen may contribute to the development of adenomyosis. This disease is the form of the chocolate cyst (endometriosis) seen in the uterus.
Treatment Methods
Adenomyosis usually resolves on its own after menopause. For this reason the treatment method is closely related to the stage of life the patient is in. Painkillers may be preferred to relieve the pain associated with adenomyosis. In particular, medications started one or two days before menstrual bleeding can prevent excessive bleeding and pain. In addition, hormonal treatments can be carried out with medications such as birth control pills containing oestrogen and progestin, or with intrauterine devices.
However, in cases where the pain is very severe and other treatment methods do not work, surgical intervention is required. Adenomyosis lesions located in a particular area of the uterus can be removed with laparoscopic or robotic surgery. This treatment is very important, especially in women who are young and considering having children. Another surgical method is the removal of the entire uterus with a hysterectomy operation. This treatment method is recommended for patients who are not considering having children in the future and for patients in whom adenomyosis involves the entire uterus.
Hysterectomy With Robotic Surgery
The removal of adenomyosis with robotic surgery is a difficult operation that must be performed by experienced surgeons. Before the operation the patient must be evaluated thoroughly and the adenomyosis border must be clearly determined. Sometimes it may be necessary to use vaginal ultrasonography during the operation so that the operation can be performed more successfully. Because the robotic arms have far greater freedom of movement in more directions than the human hand, these operations are performed very successfully. Again, the hysterectomy operation is performed robotically through 3-4 incisions smaller than 1 cm made in the patient’s abdominal region.
The robot arms and the camera are placed inside the abdomen through these incisions. Thanks to the robotic arms, the uterus is separated from its ligaments and blood vessels and removed completely. In women who are in the menopausal period, the fallopian tubes and ovaries are also removed together with the uterus. With robotic surgery the procedure takes a shorter time and the blood loss during the operation is kept to a minimum. Furthermore, because the operation is performed through small incisions, the recovery process is also considerably shortened. In this way, patients return to their normal lives much more quickly.
